About the role:
The Patient Engagement Coordinator plays a central role in ensuring that our patients receive consistent, high-quality engagement and are supported in maintaining their ongoing care. Unlike traditional call center or sales roles, this role is grounded in empathy, trust-building, and patient-centered communication.
This role will focus on reaching out to existing Accompany Health patients to schedule Annual Visits, follow-up appointments with clinicians, and provide gentle reminders, answer questions, and reinforce the value of our continuous support. Your work will help patients stay engaged with their care teams and build stronger relationships with us over time.
You’ll spend the majority of your time on outbound calls and follow-ups, but with a service mindset rather than a sales quota. Your success is measured by your ability to deliver an outstanding patient experience, ensure continuity of care, and maintain accurate records.
Responsibilities will include:
- Conduct outbound calls, texts, and other virtual outreach to schedule Annual Visits and follow-up appointments with existing patients.
- Gently educate patients on the importance of ongoing care and the benefits of completing their annual visits and any necessary follow-up appointments with clinicians.
- Handle inbound calls from patients with warmth and professionalism, answering questions and directing them appropriately.
- Maintain up-to-date and accurate documentation of all patient interactions using our internal CRM system.
- Collaborate closely with clinical care teams to ensure alignment on visit readiness and follow-ups.
- Provide an empathetic, respectful, and encouraging patient experience on every call.
- Monitor scheduling metrics and contribute to achieving engagement goals, with an emphasis on service quality over volume.
- Act as a liaison between patients and the care team to address basic concerns and questions about services.

Desired skills and experience:
- Required
- High School Diploma/GED or higher
- 2+ years of related experience in a healthcare sales, direct marketing or customer service capacity
- Ability to quickly build trust and rapport with seniors and other patients
- Ability to work after hours and weekends if needed
- Ability to work independently and as part of a team
- Are internet-connected, able to work remotely
- Preferred
- Direct-to-consumer sales or patient engagement at a health plan, medical facility or another healthcare setting
- Knowledge of Medicaid and/or Medicare programs and senior services
- Bachelor’s Degree preferred
