The Role

Are you passionate about delivering high‑quality person‑centred care in innovative settings? East Dunbartonshire is launching a new Hospital at Home Service, and we are seeking enthusiastic and dedicated Advanced Nurse Practitioners to join our values‑driven multidisciplinary team. As part of NHSGGC’s transforming together agenda, our Hospital at Home service will provide hospital‑level care to patients living with frailty in the comfort of their own home, including Care Homes. This new service will contribute to reducing unnecessary hospital admissions and support early discharge. This is a unique and exciting opportunity to be at the forefront of transforming healthcare services in East Dunbartonshire, enhancing outcomes for patients across the Health and Social Care Partnership, and contributing to service development.

Duration, Location, and Working Pattern

Permanent, Full‑time.
Shift Pattern – Monday to Sunday, 08:00–20:00.
Base location – Milngavie clinic (requirement to travel between three localities in East Dunbartonshire).

Key Responsibilities

  • Clinical assessment and diagnostic reasoning
  • Clinical judgement and decision‑making
  • Anatomy and pathophysiology
  • Non‑medical prescribing (V300)
  • Leadership in care delivery and evaluation
  • Practice‑based learning or equivalent transferable modules

Knowledge, training, qualifications and/or experience required to do the job

ANPs are educated at Masters Level in advanced practice and are assessed as competent in this level of practice. As a clinical leader, they have the freedom and authority to act and accept responsibility and accountability for those actions. This level of practice is characterised by high‑level autonomous decision‑making, including assessment, diagnosis, treatment, and prescribing of patients with complex multi‑dimensional problems.

  • Registered Nurse on the NMC Register
  • Be recorded as a Nurse Independent/Supplementary Prescriber (V300) with the NMC
  • An appropriate Master’s level qualification in Advanced Practice, enabling competence in:
  • Clinical assessment
  • Clinical reasoning, judgement and diagnostic decision‑making
  • Anatomy and pathophysiology
  • Significant comprehensive post‑registration experience within a healthcare setting, some of which must be at a senior practitioner level
  • A commitment to lifelong learning and evidence of continuing professional development
  • Experience in managing staff, leading a team, providing education and training, and establishing a supportive, learning environment
  • Successful track record in working in multidisciplinary team settings and promoting collaborative approaches to delivering excellent patient care
  • Immediate Life Support
