Requirements:

• Active LPN license in Ohio

• Reliable transportation

Responsibilities:

• Complete resident assessments and ensure compliance with state/federal care standards

• Develop and implement individualized care plans

• Support and train on Resident Rights, Infection Control, and Pharmacy processes

• Deliver compassionate, high-quality nursing care
