Title: Special Operations Case Manager / Nurse Case Manager (1-10 SFG(A), Baumholder, Germany)

We are seeking a stellar Special Operations Case Manager / Nurse Case Manager that will provide a full range of professional health nursing principles, practices, and procedures in clinical settings to analyze the full scope of problems associated with providing appropriate, cost‑effective care to Department of Defense (DOD) beneficiaries.

Essential Duties & Responsibilities

  • Collect, organize, record, and communicate data relevant to primary health assessments, including a detailed medical history, to develop time‑sensitive treatment plans for selected case‑managed patients or populations.
  • Assess patients via telephone using established protocols to provide appropriate and cost‑effective care.
  • Establish priorities for patient care, monitor and evaluate progress toward stated goals, and provide coordinated, efficient, effective health care to beneficiaries.
  • Oversee discharge‑planning activities to ensure ideal timing and sequencing of patient care.
  • Provide professional assistance to health‑care finders to identify patients’ needs for referrals to appropriate providers or facilities.
  • Negotiate, write, finalize, and administer product/service agreements to insure mutual compliance in meeting care goals.
  • Apply utilization‑review criteria to recognize and report actual or potential quality and risk management issues to improve practice and ensure compliance.
  • Use VA/DOD Clinical Practice Guidelines to ensure an evidence‑based standardized process of care for military beneficiaries.
  • Understand the healthcare financial environment to ensure optimal stewardship of resources.
  • Employ research tools to critically evaluate existing practices, opportunities, and outcomes.
  • Manage information systems and technology to oversee individual or population health, continuously improve practice, and communicate outcomes to stakeholders.
  • Utilize computers and software applications for data collection, tracking, rendering, and analysis.
  • Design and present computer‑generated outcome analysis for timely, effective peer and higher‑level educational programs or project briefings.
  • Understand pharmaceuticals, their desired effects, side effects, and complications to assess their impact on patient recovery.

Required Education/Experience/Skills/Training

  • U.S. Citizenship required.
  • Position requires a Secret or Top Secret Clearance, or the ability to obtain a clearance.
  • Demonstrated skill in understanding diagnoses to help patients optimize function and self‑care.
  • Current licensure: full, active, and unrestricted license as a Registered Nurse or Licensed Clinical Social Worker in any U.S. State, DC, Guam, Puerto Rico, or U.S. Virgin Islands.
  • Minimum two (2) years of nursing or case‑management experience; must be certified as a case manager by a recognized organization (e.g., Commission for Case Management Certification, American Case Manager Association, or American Nurse Credentialing Center) or obtain certification within the first year.
  • Knowledge of patient education principles and management of complex medical, psychosocial, and financial problems to enhance adherence to individualized treatment plans and train peers and staff.
  • Ability to seek feedback from peers, professional colleagues, clients, and outcomes research to expand clinical knowledge and enhance performance.
  • Skill to consult with health‑care providers at all levels and negotiate with outside providers for services and products to obtain client services and support.
  • May be required to receive and maintain clinical practice privileges.
  • Maintain credentialing requirements in good standing at a local MTF.
  • Fluent oral and written English to interact effectively with peers, vendors, providers, and Military Health System (MHS) stakeholders.
  • Skill in team coordination, facilitation, and communication to facilitate information exchange, education, and goal achievement.
  • Knowledge of local, state, and federal laws, professional codes of ethics, and practice standards to ensure legal and professional compliance.
  • Maintain current Basic Cardiac Life Support (BCLS) certification and provide proof of all training, certifications, and documentation.
