We are seeking a compassionate, recovery-oriented Case Manager to support individuals living with mental health challenges in a residential setting. In this role, you will use Psychiatric Rehabilitation principles to empower residents to achieve their personal recovery goals, strengthen independent living skills, and improve their overall quality of life.

Working under the direction of the Resident Manager , the Case Manager plays a key role in creating a safe, supportive, and recovery-focused environment while collaborating with residents, families, team members, and community partners. Key Responsibilities

Utilize Psychiatric Rehabilitation principles to promote recovery, community integration, and improved quality of life.
Develop trusting and supportive relationships with residents while maintaining professional boundaries.
Support residents in achieving individualized recovery goals and participating actively in their recovery process.
Encourage self-determination, self-help skills, and independence.
Provide emotional support and guidance as needed.
Advocate for residents and promote self-advocacy opportunities.

Daily Living & Residential Support

Educate and assist residents with daily independent living activities (IADLs).
Foster a supportive, collaborative, and recovery-oriented home environment.
Assist with light housekeeping and household support activities.
Help residents develop natural support systems within the community.

Team Collaboration

Collaborate with treatment team members, families, significant others, and community providers.
Assist staff and service providers in understanding and implementing Psychiatric Rehabilitation practices.
Support the overall mission, values, and policies of RHD.
Ensure operational procedures are followed and contribute to effective program operations.

Qualifications Required

Bachelor's Degree in a Human Services-related field.
Valid Driver's License.
Strong interpersonal, communication, and problem-solving skills.
Commitment to recovery-oriented and person-centered care.

Preferred

Experience working in behavioral health, residential services, case management, or psychiatric rehabilitation.
Knowledge of recovery-focused practices and community resources.

Physical Requirements

This position requires the ability to:

Walk, stand, and sit throughout the workday.
Climb stairs as needed.
Reach, bend, stoop, kneel, crouch, and crawl.
Push and pull equipment or supplies.
Perform repetitive movements with hands and arms.
Communicate effectively and observe resident needs and environmental conditions.

Lifting Requirements

Medium Duty Work

Lift up to 50 pounds occasionally.
Lift up to 20 pounds frequently.
Lift up to 10 pounds continuously.

Work Environment

This position may involve exposure to:

Dust, fumes, mists, and gases.
Extreme temperatures, humid or wet conditions.
Loud noise levels.
Confined spaces.
Mechanical, chemical, electrical, and burn hazards.
