We are seeking a compassionate and motivated Registered Nurse to join the District Nursing Service. Working alongside the District Nurse Sister/Charge Nurse, you will deliver high-quality, person-centred nursing care to patients within the community. You will provide skilled nursing interventions, support patients and their families to maintain independence, and work collaboratively with a range of health and social care professionals. This role requires the ability to work independently and autonomously while contributing to a responsive service that operates 365 days a year and supports patients with a wide range of complex health needs.

Monday to Sunday, Weekends, Public Holidays and Out of Hours
The post holder will assess, plan, implement and evaluate programmes of care for patients within the district nursing caseload, ensuring care is evidence-based, person-centred and developed in partnership with patients, families and carers. Working under the direction of the District Nurse Sister/Charge Nurse, you will deliver skilled clinical interventions, manage and prioritise your own workload, support the effective use of resources, and contribute to the delivery of a high-quality 24-hour nursing service. You will provide clinical leadership, support junior staff and students, participate in audits, research and quality improvement activities, maintain accurate clinical records, comply with professional and organisational policies, and work collaboratively with multidisciplinary teams and partner agencies to ensure the best outcomes for patients. The role also involves responding appropriately to emergency situations, promoting health and wellbeing, supporting discharge planning, developing care packages, and contributing to the continuous improvement of district nursing services.
