Community Staff Nurse - North West

The Role

We are seeking a compassionate and motivated Registered Nurse to join the District Nursing Service. Working alongside the District Nurse Sister/Charge Nurse, you will deliver high-quality, person-centred nursing care to patients within the community. You will provide skilled nursing interventions, support patients and their families to maintain independence, and work collaboratively with a range of health and social care professionals. This role requires the ability to work independently and autonomously while contributing to a responsive service that operates 365 days a year and supports patients with a wide range of complex health needs.

4 x Permanent Roles (3 x 36 Hours & 1 x 21.6 Hours)

Possilpark Health and Care Centre & Partick Community Centre for Health

Monday to Sunday, Weekends, Public Holidays and Out of Hours

Key Responsibilities

The post holder will assess, plan, implement and evaluate programmes of care for patients within the district nursing caseload, ensuring care is evidence-based, person-centred and developed in partnership with patients, families and carers. Working under the direction of the District Nurse Sister/Charge Nurse, you will deliver skilled clinical interventions, manage and prioritise your own workload, support the effective use of resources, and contribute to the delivery of a high-quality 24-hour nursing service. You will provide clinical leadership, support junior staff and students, participate in audits, research and quality improvement activities, maintain accurate clinical records, comply with professional and organisational policies, and work collaboratively with multidisciplinary teams and partner agencies to ensure the best outcomes for patients. The role also involves responding appropriately to emergency situations, promoting health and wellbeing, supporting discharge planning, developing care packages, and contributing to the continuous improvement of district nursing services.

Knowledge, training, qualifications and/or experience required to do the job

• Registered Nurse with current NMC registration.
• Knowledge of the NMC Code, legal and ethical frameworks, and relevant healthcare legislation.
• Understanding of district nursing practice and community-based care delivery.
• Knowledge of health promotion, health improvement and the determinants of ill health.
• Ability to assess, plan, implement and evaluate patient care programmes.
• Experience of managing patients with complex healthcare needs, including palliative and chronic disease care.
• Understanding of risk assessment processes and incident reporting procedures.
• Knowledge of local, regional and national policies, procedures and clinical guidelines.
• Competence in maintaining accurate clinical records in line with NMC standards and data protection requirements.
• Basic Life Support skills and ability to respond to emergency situations.
• Commitment to continuous professional development and participation in mandatory training.
• Experience of supporting, supervising or mentoring students and junior staff.
• Understanding of clinical audit, quality improvement and evidence-based practice.
• Effective communication, team working and partnership-building skills across healthcare agencies.
