Qualifications and Requirements:

• Obtain patient information as needed
• Evaluate and assess documentation to ensure accuracy and compliance
• Ability to effectively communicate with office and clinical teams
• Home health or medical intake experience preferred
• Ability to successfully complete a background check and professional reference check
