Position Title: Nurse Practitioner 100% Virtual, CareBridge

Sign-on Bonus: $5,000

Eligibility: Licensed Nurse Practitioner in Ohio with an active RN Compact license.

Location

Virtual (eligible candidates must have an active RN Compact license; candidates within commuting distance may be considered).

Work Shift

Monday – Friday, 8:00 am to 5:00 pm EST with rotating on‑call 2–3 days per month from 5:00 pm to 8:00 am EST.

Responsibilities

• Provide urgent health care via telephone and tele‑video modalities to patients receiving home and community‑based services through state Medicaid programs, dual‑eligible members, and other memberships as assigned by our MCO partners.
• Develop and implement clinical plans of care for adult patients with chronic and complex conditions.
• Gather history, physical exam, and diagnostics as needed, then develop and implement treatment plans in accordance with patient goals and current conditions.
• Identify and close gaps in care.
• Meet the patient’s and family’s physical and psychosocial needs with support from the company’s inter‑disciplinary team.
• Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention.
• Maintain contact with other clinical team members and external providers to coordinate optimal care and resources for patients in a timely manner consistent with state regulations and company policy.
• Maintain patient medical records and documentation in compliance with state regulations and company standards.
• Participate in continuing education as required by state and certifying bodies.
• Prescribe medication as authorized by state prescribing authority.

Minimum Requirements

• MS in Nursing.
• Active, national NP certification.
• Valid, current, active, and unrestricted family or adult NP license in the state of Ohio.
• Valid, current, active compact RN license.
• 2+ years of experience managing complex care cases.
• Experience working with Electronic Medical Records (EMR).

Preferred Skills, Capabilities and Experiences

• Active RN Compact license.
• Possession of DEA registration or eligibility preferred.
• Active Medicaid number in the state of Ohio highly preferred.
• Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred.
