Registered Nurse, Case Manager

Full‑time opportunity that plays a key role in supporting patients throughout their hospital stay by ensuring coordinated, high‑quality care.

Job Responsibilities

• Provide comprehensive care coordination for hospitalized patients.
• Conduct high‑risk screenings and assessments to identify patient needs.
• Arrange and document post‑acute care services, ensuring smooth transitions of care.
• Collaborate with a multidisciplinary healthcare team to support optimal patient outcomes.

Experience Requirements

• 1 year case management experience

Education & Credential Requirements

• Associate of Science in Nursing required
• Bachelor of Science in Nursing preferred
• Cardiopulmonary Resuscitation Certification (CPR) required
• Licensed Registered Nurse required
