SUD Nurse Practitioner

Stellenbeschreibung:

Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning
Diagnose health conditions and create individualized treatment plans
Prescribe and manage medications, monitor for side effects, and adjust treatment as needed
Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care
Serve as a clinical consultant on complex or high-need cases
Educate clients and families on treatment plans, medications, and recovery support strategies
Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards
Participate in team meetings, professional development, and continuous training
Provide telehealth-based assessments, medication management, and care coordination

Requirements

Active and unrestricted NP license in Washington
Active DEA license (and CDS where applicable)
Several years of experience in behavioral health, addiction medicine, or psychiatry
Experience in addiction medicine or psychiatry
Board certification in specialty preferred
Strong documentation skills
Proficiency in EHR platforms
Deep compassion toward individuals living with SUD and mental health challenges
Ability to collaborate in integrated and team-based care models
Ability to navigate complex systems
Clear and confident virtual communication skills
