Virtual Clinician, Home Evaluations, Nurse Practitioner

Responsibilities

• Conduct online “Video Visits” with health plan members
• Engage with health plan members primarily online, helping to close gaps in care
• Provide 40 hours of availability weekly
• Execute a strong visit completion rate
• Maintain compliance with operational, clinical, and member experience key performance indicators

Requirements

• Full‑time, board‑certified nurse practitioners or physician assistants
• Experience with Signify Health In‑Home Health Evaluations, Video Visits, telehealth visit delivery
• Maintain training, affiliation, and compliance with Video Visit requirements
• Be trained and credentialed for all health plan clients in primary coverage state(s)
• Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs)
• Able to obtain and maintain multi‑state licenses as required

Demonstrates expertise in conducting Video Visits and telehealth delivery while maintaining compliance with clinical and operational standards. Holds active state licenses and certifications, ensuring high‑quality care and member engagement.
