About the Role:

• We are looking for a Care Navigator to help patients navigate complex systems and overcome barriers to care.
• This role involves high-volume outreach and end-to-end coordination for services like mammograms and primary care.
• You will also connect patients to social and community resources that impact their ability to receive care.

Key Responsibilities:

• Conduct outbound calls to engage patients and schedule services.
• Coordinate care across multiple services and address social determinants of health.
• Maintain accurate documentation and collaborate cross-functionally.
