Community Mental Health Nurse for Physical Health Care B6

The Role

West Dunbartonshire Older Adult Community Mental Health Services are recruiting a Band 6 Community Psychiatric Nurse (CPN) to support and improve physical health care outcomes for patients with complex mental health needs. The post holder will play a key role in delivering and maintaining the Physical Health Care service across the two Community Mental Health Teams in West Dunbartonshire, ensuring compliance with the NHSGGC Physical Health Care Policy.

All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.

Base location, working over 2 sites - Cairnmhor Resource Centre and Dumbarton Hospital

Key Responsibilities

• Ensure local compliance with NHSGGC Physical Health Care Policy protocols and systems.
• Deliver physical health care services to patients with complex mental health needs.
• Work collaboratively with Adult Community Mental Health Team staff, GPs, practice nurses, and third-sector partners.
• Deliver a person-centred and strengths-based approach to care.
• Run local Physical Health Care Clinics across two sites.
• Work independently without direct supervision.
• Support and supervise junior nursing colleagues.
• Promote and develop effective partnership working with patients and their carers.
• Provide care and support in a range of community settings, including patients' homes when required.

Knowledge, training, qualifications and/or experience required to do the job

• Knowledge of the NHSGGC Physical Health Care Policy and its local implementation requirements.
• Understanding of physical health care needs within older adult mental health services.
• Ability to work within multidisciplinary and multi-agency partnerships.
• Skills in delivering person-centred and strengths-based care.
• Experience or ability to supervise and support junior nursing staff.
• Competence in managing and delivering clinics independently.
