Job Overview
This position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients. It involves coordinating care among the patient, family, physicians, providers, care teams, community resources, and the Regional Operations Manager.
Qualifications
- A.D.N degree from an accredited school of nursing required; B.S.N preferred; three‑year diploma from an accredited program may be substituted for a nursing degree.
- Current RN license in the state of practice.
- Continuing education credits maintained as required by state practice.
- Minimum of five years’ experience in clinical nursing.
- Minimum of three years’ experience in renal nursing preferred.
- Case‑management experience required; alternatively 3+ years of experience assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex healthcare transitions.
- Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred.
- Demonstrated knowledge of data and ability to manage clinical, financial, and patient satisfaction outcomes.
- Evidence of effectiveness in a change‑agent role.
- Knowledge of continuous quality improvement techniques.
- Prior experience in healthcare performance coaching required.
- Current CPR certification required.
- Ability to adapt personal practice patterns to on‑board electronic processes and productivity expectations related to Capella implementation.
- Valid driver’s license with a positive driving record and meeting insurance coverage requirements.
- Basic computer skills and proficiency in MS Word and Outlook.
- Functional proficiency with DaVita specific clinical software, including Capella, required within 90 days of employment.
- Home office with internet connectivity of at least 1 MB upload and 1 MB download speed required.

Salary
Base salary range: $68,400.00 – $100,400.00 per year, based on qualifications, skills, competencies, and experience.
