Advanced Practitioner: North East Rehabilitation Service

We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

This permanent full‑time post is 37 hours per week and will be based between Petershill Park Business Centre, 28–30 Adamswell Street, Glasgow, G21 4DD and Parkhead Hub, 1251 Duke Street, Glasgow, G31 5NZ.

The shift pattern for this post is 5/7 days, including weekends and public holidays.

Overview

An exciting opportunity has arisen for an Advanced Practitioner to join the North East Rehabilitation Service. This developing role requires a proactive, independent, enthusiastic person who enjoys building networks and developing roles and pathways.

Responsibilities

  • Assess, diagnose and manage the clinical needs of a wide cohort of patients autonomously and with other members of the interdisciplinary team, including the management of a defined caseload of patients.
  • Refer, admit and discharge within appropriate clinical areas.
  • Care coordination and liaison with acute, primary care and social work colleagues.
  • Ensure delivery of evidence‑based care.
  • Provide clinical and professional leadership, management and advice to other multidisciplinary team members.
  • Work autonomously within a multidisciplinary team.
  • Set, implement and evaluate standards of practice.

Qualifications

  • Registered Nurse or Allied Health Professional with significant comprehensive post‑registration experience within a healthcare setting, some of which must be at a senior practitioner level.
  • Appropriate (or working towards) Masters level qualification in Advanced Practice.
