Director, Community Health Operations (Fully Remote)

• Owns the operational backbone of community-based care: the CHW workforce and Community Supports.
• Leads two teams through two leaders across twelve counties and eight health plan contracts.

• Deployment, hiring, and integration of community teams with clinical teams.
• Audit accountability, performance reporting, and unit economics for Community Supports.
